Our Services for People with Disabilities team is searching for a dedicated, energetic full-time Contract Monitor for our Host Home Services in the Northwest Iowa region. LSI has a presence in Northwest Iowa and would be open to office location in one of those areas. (Ames, Mason City, Denison, Spencer, Sioux City or Storm Lake) This position does have some flexibility to work from home as well. Duties The Contract Monitor oversees specified Host Homes in the Northwestern Iowa area, and is responsible for the oversight of services and communicating with the Independent Providers. This role is responsible for: - Acting as the main contact for interdisciplinary team members and for consumers in the sites - Working directly with consumers to provide support, assistance, and instruction in areas which facilitate personal growth and independence This position may require long hours including some evenings and weekends to ensure the safety and satisfaction of the individuals served. Will also be part of an on-call rotation. The Northwest Region covers the following 33 counties: Story, Hardin, Franklin, Cerro Gordo, Worth, Winnebago, Hancock, Wright, Hamilton, Boone, Greene, Webster, Humboldt, Kossuth, Emmet, Palo Alto, Pocahontas, Calhoun, Carroll, Crawford, Sac, Buena Vista, Clay, Dickinson, Osceola, O'Brien, Cherokee, Ida, Crawford, Monona, Woodbury, Plymouth, Sioux, & Lyon. Qualifications Qualified candidates for this job will have one of the following: - A Bachelor's degree in Human Services, Education, or another human services-related field, plus two years of full-time, work related experience - A Master's degree in Social Work or another human services-related field, plus one year of full-time, work-related experience This position requires a valid driver's license, auto insurance, a good driving record, and the ability to pass an extensive background check, including checks of the Dependent Adult and Child Abuse registries, the Sex Offender registry, and the criminal history check.
