The LINKS Coach is responsible for the supervision and care of individuals attending the LINKS Day Habilitation Program. The LINKS Coach will daily organize and prepare for the activities, outings and personal cares of individuals under the direction of the Team Coordinator. The LINKS Coach reports directly to the LINKS Team Coordinator or the ES & LINKS Team Coordinator.
Essential Duties
This position includes, but is not limited to, the following essential functions:
Assist in coordinating all aspects of program to meet standards of all regulatoryAgencies such as: CARF, Department of Inspections and Appeals, Department of Labor, Management Care Officer and OSHA
Work cooperatively with the person served, their service coordinator, and other team members to support the persons served "Person-Centered Plan"
Assist persons served with specific individual needs
Set up the activity area daily, ensuring it is ready for activities to begin
Assist with supervision of persons served during break and meal time
Maintain a clean, healthy, and safe environment for persons served
Provide physical assistance to persons served as necessary in the areas of eating,dressing, personal hygiene, etc.
Document persons served interactions and support interventions in the form of contact notes/Therap ISP data, incident reports, T-log timely and as instructed.
Oversee, and participate in, the clean up each day before everyone leaves
Participate in Person-Centered Plan when asked
Keep direct supervisor informed of persons served needs and progress
Assist persons served in activities such as community outings, socialization, following directions, attention to tasks, job readiness and making crafts
Model for or physically assist persons served with actual activities and/or tasks
Lead small group activities
Must have authorized release before sensitive information is given pertaining to CWI, persons served, and staff
Will be aware of and comply with all HIPAA regulations in coordination with the HIPAA Compliance Officer
Will abide by all CWI Corporate Compliance policies and Code of Ethics
Comply with instructions and directions given by direct supervisor for person served individual and group needs
Provide services perapproved policies and procedures, ensuring appropriate implementation of Person-Centered Plan
Maintain files in a neat and orderly fashion and assure limited access to such (on the computer as appropriate)
Implement, evaluate and coordinate each person served goals
Implement or see to the implementation of behavioral plans
Work a flexible schedule to accommodate the Company's needs as applicable
Other duties as assigned by supervisory personnel.

Vision abilities required by this job include closeand farvision.
Working Conditions
Working conditions vary from indoors activities to outdoor activities. The noise level in the work environment is usually quiet to moderate, but occasionally can be noisy.The employee must be able to travel between locations, and occasionally attend seminars which may require an overnight stay.
Requirements/Qualifications
To perform the LINKS Coach job successfully, the individual must be able to perform each essential responsibility satisfactorily. Reasonable accommodations may be made to enable individuals to perform the essential functions of the position.
The LINKS Coach will have the following experience and attributes:
High School diploma or GED.
A valid Chauffeur's license with a driving record that is acceptable to CWI's auto insurance company
